*** Lab Services: You may receive a bill from an outside laboratory company for services we are not able to process in our in-house lab.

Reminder

Please bring a physical copy of your insurance card to every visit.

All deductible, coinsurance, and past due balances are due at the time of service.

Office visits will be assessed an estimated charge for those with high deductible plans and coinsurances based on our contracted rates with your insurance carrier.

Failure to provide 24-hour notice on appointment cancellations will result in a $50 no-show fee for office visits, a $100 no-show fee will be assessed for office procedures and surgeries.

We are currently not accepting any new Medicaid, Dual, or Medicaid Managed Care patients.

The only Essentials Product AWM participates with is Excellus.

Existing patients enrolled in our Patient Portal can submit questions through the “Ask a Billing Question” feature.
